Once a patient is discharged, they become part of the Alumni experience. Using a Salesforce integration and the custom-branded TTC Healthcare mobile app, Transformations shares 12 automated surveys over 12 months to interact with Alumni and keep them connected and engaged in the recovery process.

If an Alumnus goes back into treatment, the surveys are triggered to start up again. After one year of recovery, Alumni will automatically receive new surveys every 6 months, helping clients stay in recovery for a lifetime.
